They head to Halstead Town needing to win and hope leaders Little Oakley, who are two points above the east Londoners, do not beat Basildon Town.
Hamlets hit White Ensign for six last weekend, after a 1-0 midweek win over Barking.
Shomari Barnwell opened the scoring at Mile End Stadium on 15 minutes, with Michael Agboola doubling the advantage in first-half stoppage time.
Steven Carvell made it 3-0 within a minute of the restart, before Barnwell bagged his side moments later.
Phil Brown-Bampoe got in on the act to make it 5-0 on 73 minutes, with Samir Bihmoutine - the midweek matchwinner - completing the tally soon after.
Should Hamlets make it three wins in a row at Halstead and Oakley suffer a defeat, they will be champions.
If Oakley draw and Hamlets win by three goals they will finish top on goal difference.
But if Hamlets lose and Great Wakering win at Ilford, they would drop down to third place in the final table.
Play-off semi-finals will then be held in midweek.
It's tight at the other end of the table, with Sporting Bengal United climbing out of the bottom two after a 2-0 win at Hackney Wick.
Samuel Adetiba and Joshua Jackson netted in each half to seal the points, as rivals Athletic Newham lost at bottom club Frenford to drop into the relegation zone.
